The Canadian Hospice Palliative Care Association has released a campaign kit for National Advance Care Planning Day on April 16, 2018. This year’s kit includes templates for announcing ACP Day in emails, news releases, newsletters, social media, etc., as well as guides, posters, pamphlets, and an inventory of some of our best videos on ACP.
